Pulmonary embolism (PE) is a potentially fatal condition that presents with shortness of breath and chest pain. Although an electrocardiogram (ECG) may show changes associated PE, ST elevation mimicking myocardial infarction not common in patients acute PE. This presentation documents the case 67-year-old man who presented to emergency department dyspnea ECG data indicated anterior septal ischemia lower parts left ventricle. Cardiac catheterization did reveal atherosclerotic lesions coronary arteries, but revealed anomaly filling right artery embolic masses. Further examination patient massive bilateral pulmonary embolism. In this presentation, we emphasize need evaluate PE present pain, dyspnea, or both, even when findings suggest cardiac etiology.
